There are some headers in libc that require us to have definitions,
such as `FILE` available to us (such as in `pwd.h`). It is bad
practice to include the entirety of `stdio.h`, so it makes more
sense to put `FILE` into it's own header.
As suggested by Joshua, this commit adds the 2-clause BSD license as a
comment block to the top of every source file.
For the first pass, I've just added myself for simplicity. I encourage
everyone to add themselves as copyright holders of any file they've
added or modified in some significant way. If I've added myself in
error somewhere, feel free to replace it with the appropriate copyright
holder instead.
Going forward, all new source files should include a license header.
The "stddbg" stream was a cute idea but we never ended up using it in
practice, so let's simplify this and implement userspace dbgprintf() on top
of a simple dbgputch() syscall instead.
This makes debugging LibC startup a little bit easier. :^)
